SERVICE WILL :
FACILITATE EARLY IDENTIFICATION

TIMELY ASSESSMENT

SHORT TERM INTERVENTION ,REFFERALS FOR PI CHILDREN, YOUNG PEOPLE, WHOSE FAMILY LIFE CIRCUMSTANCE PUT AT RISK.

SERVICE WILL BE SHARED BETWEEN THE TONGAN HEALTH SOCIETY AND MT ROSKILL POLICE COMMUNITY PROJECT.
 
 
 
 WHANAU FAMILY CARE & SUPPORT PLAN (PROJECT)
 TARGET AGE GROUP : 0 – 5 YEARS
CASE FINDING : Passive Case detection.

Family Care and support service (0 – 5 yrs old) are one of a number of service identified under the Frame work of Well child care (WCC) services which have been developed in response to the Child Health Strategy
SERVICE OBJECTIVES :
  • To work with those families at highest risk ,of not accessing health service
  • Service will ensure that the families must take on-going responsibilities to access all scheduled health checks, as well as scheduled immunizations.

 
WELL CHILD SERVICE PLAN. 
TARGET AGE GROUP: 0 – 5 years.
 CASE FINDING : Passive and active Case detection
 CASE LOAD : 400 Cases.

PURPOSE :
  • To identify as early as possible those Children not likely to access Tamariki Oral/Wcc and immunisation Service.
  • To provide them with way of receiving them

SERVICE OBJECTIVE :

  • To work with Family/Whanau
  • To ensure that they understand the value of well child services
  • To ensure children are registered with a G.P
  • To ensure Families take on –going  responsibilities to access all scheduled Checks.
  • An average of 13 visits (i.e. 2.6 visit per child per year) will be sufficient
  • If more than it is required – Family should referred to a Family Whanau Care and Support Service.

COMMUNITY BASED ASTHMA SERVICE (PROJECT) 
TARGET AGE :     Group 0 – 15 years.
CASE FINDINGS : Passive and active Case detection
SERVICE OBJECTIVE :
Core Service of the Community based asthma care includes :
  • Prevention for low at risk children and young people with asthma including population health education and promotion.
  • Diagnosis,treatment,medication advice and educate PI children and young people with asthma
  • Introductory visit on asthma patients in medical and pediatrics wards and post discharge visits when required.
  • Assist in developing self management plans for individual and families who are at risk with asthma.
  • Educate and support for PI children and young people with asthma
  • Referral to course as appropriate to asthma disease e.g smoking cessation course
 
 
YOUTH AT RISK SERVICES.(PROJECT)
TARGET AGE GROUP 10 – 19 YRS OLD
CASE FINGDINGS:  Passive Case Detection.

Purpose

The services to be provided will facilitate early identification, timely assessment, short term intervention and referrals, for Pacific young people whose social and family life circumstances put at risk positive life outcomes.

The service includes:
  • Mental health issues, including drug and alcohol use; grief and loss; suicide identification; anxiety; eating disorder; body image.
  • Personal health issues, including sexuality and sexual health.
  • Social issues including family issues; relationship difficulties; social isolation; abuse access to education (safe program).
  • It will include support, co-ordination, promotion of self-worth, self-management of clinical/medical conditions and healthy life-style.

SCHOOL BASED NURSING HEALTH SERVICE
Target age Group : 6 -13 years.
    Case Finding : Passive Case detection.

  • The purpose of the School based nursing service (service plan)
  • To work with School Staff
  • Relevant community organization to identify as early as possible those children who have not or are likely to access health care service.

Service Objectives
  • Service will develop school health plans in Conjunction with the school
  • Service work with other provider e.g. Public Health Nurse, Health Promotion service, G.P
SERVICE USERS :
    The intensive school health service is aimed at improving the Health outcome for the tamariki children requiring additional health service in decile (1) school.
 
 

 MOBILE NURSING SERVICE – SERVICE PLAN.

Target Age Group : 10 – 19 Years
Case Finding : Passive Case detection.
Performance required 1st.Quarter : Increase 20% new case finding
= 69
Performance achievement 1st.Quarter : 93
Number of Nurses = 1
 
 

OUTREACH IMMUNISATION SERVICES (OIS)  (PROJECT)
TARGET AGE GROUP : 0 – 5 YEARS
CASE FINDINGS : Passive and Active case detection.

PURPOSE :
To ensure Maori, Pacific and other priority groups with high rates of vaccine preventable disease and low rate of immunization have access to the (OIS)

SERVICE USERS :
(OIS) are for the children and families/whanau who are not accessing immunization service
