At Tongan Health Society, we understand the unique needs of our Pacific elders and disabled people. Our Homecare Navigation service is designed to ensure our elders and disabled community receive the best possible care, helping them to live comfortably and independently in their own homes.
We are aware that it can be hard for carers and our elders and disabled community to know where to go to find out what support services are available.
We aim to provide a navigation pathway for our Pasifika community to access the services that they are eligible for and need.

Tui mo e ‘Amanaki Navigation Service
Step 1 - Patient Engagement
The process begins when the elders, disabled community or family initiates contact with Tui mo e ‘Amanaki.
Step 2 - Initial Assessment
Tui mo e ‘Amanaki conducts a comprehensive assessment that includes cultural, health, and personal needs. We can provide support accessing Incontinence Products, Personal Care Support, Mobility Aids, Carer Support, Hospital Beds, Respite.
Step 3 - Care Plan Implementation
Based on the assessment, a personalised care plan is developed in consultation with the elder, disabled person and family.
Step 4 - Service Coordination
Tui mo e ‘Amanaki coordinates with
various healthcare providers and community services.
Step 5 - Service Delivery
The elders and disabled community receives the agreed-upon services.
Step 6 - Ongoing Monitoring
Tui mo e ‘Amanaki regularly monitors the elders and disabled person’s condition
and satisfaction with services.
Step 7 - Review and Adjut
The care plan is reviewed periodically,
and adjustments are made as needed.
The process is continuous, with the cycle repeating as necessary to ensure ongoing support and care for the elderly and disabled community.
